Tav and her lover Gale just want peace and quiet, but she is worried about Astarion and realises after they loose the tadpoles he won't be able to stay in sunlight, and will loose his safe supply of blood (her). She loves him and doesn't want him fleeing as soon as the battle is won.
Post-battle, and after a tearful goodbye to Karlach as she and Wyll go to Avernus, and just as Astarion starts burning in the sun Tav and/or Gale teleports Astarion inside Gale's Waterdeep tower. Astarion is confussed - he assumed he'd just go to the Underdark and start sucking on rats again. Tav tells him he's to stay with her and Gale while they find a cure for vampirism - and for Karlach's engine. Gale managed to never leave his tower so Astarion will be safe. He can drink Tav's blood every night. Astarion is shocked and overwhelmed by her (and Gale)'s kindness.
Astarion, Gale and Tav end up in a happy, committed and consensual poly relationship (all three are in love with each other) that gradually heals Astarion body and soul. It also turns out the trio are a force to be reckoned with.
Bonus: Astarion, Gale & Tav have a wedding for all three of them and all companions turn up They find a cure for Karlach's engine They find a cure for vampirism and help all Cazador's spawn
